Meeting nutritional targets of critically ill patients by combined enteral and parenteral nutrition: review and rationale for the EFFORTcombo trial

While medical nutrition therapy is an essential part of the care for critically ill patients, uncertainty exists about the right form, dosage, timing and route in relation to the phases of critical illness. As enteral nutrition (EN) is often withheld or interrupted during the intensive care unit (ICU) stay, combined EN and parenteral nutrition (PN) may represent an effective and safe option to achieve energy and protein goals as recommended by international guidelines. We hypothesise that critically ill patients at high nutritional risk may benefit from such a combined approach during their stay on the ICU. Therefore, we aim to test if an early combination of EN and high-protein PN (EN+PN) is effective in reaching energy and protein goals in patients at high nutritional risk, while avoiding overfeeding. This approach will be tested in the here-presented EFFORTcombo trial. Nutritionally high-risk ICU patients will be randomised to either high (≥2·2 g/kg per d) or low protein (≤1·2 g/kg per d). In the high protein group, the patients will receive EN+PN; in the low protein group, patients will be given EN alone. EN will be started in accordance with international guidelines in both groups. Efforts will be made to reach nutrition goals within 48–96 h. The efficacy of the proposed nutritional strategy will be tested as an innovative approach by functional outcomes at ICU and hospital discharge, as well as at a 6-month follow-up.

Impulsive aggressive (IA, or impulsive aggression) behavior describes an aggregate set of maladaptive, aggressive behaviors occurring across multiple neuropsychiatric disorders. IA is reactive, eruptive, sudden, and unplanned; it provides information about the severity, but not the nature, of its associated primary disorder. IA in children and adolescents is of serious clinical concern for patients, families, and physicians, given the detrimental impact pediatric IA can have on development. Currently, the ability to properly identify, monitor, and treat IA behavior across clinical populations is hindered by two major roadblocks: (1) the lack of an assessment tool designed for and sensitive to the set of behaviors comprising IA, and (2) the absence of a treatment indicated for IA symptomatology. In this review, we discuss the clinical gaps in the approach to monitoring and treating IA behavior, and highlight emerging solutions that may improve clinical outcomes in patients with IA.

Objectives
The national implementation of competency-based medical education (CBME) has prompted an increased interest in identifying and tracking clinical and educational outcomes for emergency medicine training programs. For the 2019 Canadian Association of Emergency Physicians (CAEP) Academic Symposium, we developed recommendations for measuring outcomes in emergency medicine training in the context of CBME to assist educational leaders and systems designers in program evaluation.
MethodsWe conducted a three-phase study to generate educational and clinical outcomes for emergency medicine (EM) education in Canada. First, we elicited expert and community perspectives on the best educational and clinical outcomes through a structured consultation process using a targeted online survey. We then qualitatively analyzed these responses to generate a list of suggested outcomes. Last, we presented these outcomes to a diverse assembly of educators, trainees, and clinicians at the CAEP Academic Symposium for feedback and endorsement through a voting process.
ConclusionAcademic Symposium attendees endorsed the measurement and linkage of CBME educational and clinical outcomes. Twenty-five outcomes (15 educational, 10 clinical) were derived from the qualitative analysis of the survey results and the most important short- and long-term outcomes (both educational and clinical) were identified. These outcomes can be used to help measure the impact of CBME on the practice of Emergency Medicine in Canada to ensure that it meets both trainee and patient needs.
Microstructure evolution and thermal properties of an additively manufactured, solution treatable AlSi10Mg part

Because of rapid solidification involved in the laser or e-beam based additive manufacturing (AM) process, solution treatable metallic parts made by these methods usually possess a unique nonequilibrium microstructure which changes significantly during subsequent thermal treatment. Such evolution alters the size, morphology, length scale, and distribution of microstructural features and has a substantial impact on thermal properties and possibly on electrical properties as well. This study focuses on effects of microstructural evolution on thermal properties of an additively manufactured AlSi10Mg part. The changes of thermal properties such as thermal expansion, heat capacity, thermal diffusivity, and thermal conductivity as a function of thermal treatment are reported. The results show that the formation of supersaturated primary α aluminum and unique cellular structure imparted by fast solidification in the AM process are the major cause for the low thermal diffusivity and low thermal conductivity observed in this solution treatable, as-built part. A correlation between microstructural evolution and changes in thermal properties is established. Advantages and tailoring of the thermal properties of additively built parts are discussed. Implications of these results are important for other additively manufactured components based on popular solution treatable alloys.
Dynamic changes in white matter microstructure in anorexia nervosa: findings from a longitudinal study

Background
Gray matter (GM) ‘pseudoatrophy’ is well-documented in patients with anorexia nervosa (AN), but changes in white matter (WM) are less well understood. Here we investigated the dynamics of microstructural WM brain changes in AN patients during short-term weight restoration in a combined longitudinal and cross-sectional study design.
MethodsDiffusion-weighted images were acquired in young AN patients before (acAN-Tp1, n = 56) and after (acAN-Tp2, n = 44) short-term weight restoration as well as in age-matched healthy controls (HC, n = 60). Images were processed using Tract-Based-Spatial-Statistics to compare fractional anisotropy (FA) across groups and timepoints.
ResultsIn the cross-sectional comparison, FA was significantly reduced in the callosal body in acAN-Tp1 compared with HC, while no differences were found between acAN-Tp2 and HC. In the longitudinal arm, FA increased with weight gain in acAN-Tp2 relative to acAN-Tp1 in large parts of the callosal body and the fornix, while it decreased in the right corticospinal tract.
ConclusionsOur findings reveal that dynamic, bidirectional changes in WM microstructure in young underweight patients with AN can be reversed with brief weight restoration therapy. These results parallel those previously observed in GM and suggest that alterations in WM in non-chronic AN are also state-dependent and rapidly reversible with successful intervention.

The powder-bed laser additive manufacturing (AM) process is widely used in the fabrication of three-dimensional metallic parts with intricate structures, where kinetically controlled diffusion and microstructure ripening can be hindered by fast melting and rapid solidification. Therefore, the microstructure and physical properties of parts made by this process will be significantly different from their counterparts produced by conventional methods. This work investigates the microstructure evolution for an AM fabricated AlSi10Mg part from its nonequilibrium state toward equilibrium state. Special attention is placed on silicon dissolution, precipitate formation, collapsing of a divorced eutectic cellular structure, and microstructure ripening in the thermal annealing process. These events alter the size, morphology, length scale, and distribution of the beta silicon phase in the primary aluminum, and changes associated with elastic properties and microhardness are reported. The relationship between residual stress and silicon dissolution due to changes in lattice spacing is also investigated and discussed.

Systematic first-principles calculations on transition metal (TM) impurities of the 3d series in Si have been performed. The equilibrium sites, migration energies, electrically-active gap levels, charge and spin states are predicted. While the properties of the isolated interstitials are experimentally well-known, much less experimental information is available about the consequences of their interactions with vacancy-like defects. We discuss here the properties of isolated interstitial Ti, Fe, and Ni, their interactions with vacancies and divacancies, the properties of the resulting substitutional impurities, and of the TM-divacancy {V-TM-V} complexes. In equilibrium, interstitial Ti, Fe, and Ni do not become substitutional, but a number of processing steps commonly used in PV manufacturing introduce highly mobile vacancies into the bulk. These vacancies strongly interact with interstitial TMs. At the substitutional site, Ti, Fe, and Ni have very different electrical properties than at the tetrahedral interstitial site. In particular, the electrical activity (and stable spin state) of Ti and Fe are greatly reduced, suggesting that the passivation by vacancies plays an unrecognized role during a variety of high-temperature processes.

Hydrogen is commonly used to remove (or at least reduce) the electrical activity of numerous defects and impurities in Si. Although hydrogenation works quite well for many defects, it has generally been unsuccessful with transition metal (TM) impurities. A number of {TM,Hn} complexes have been detected using optical or electrical techniques. Even though the gap levels of the isolated TM shift upon hydrogenation, many {TM,Hn} complexes remain electrically active. The nature of the complexes responsible for specific DLTS lines is generally not known, and the number of H interstitials in a given complex is assumed. We have performed systematic first-principles calculations involving Ti-H and Ni-H interactions in Si, assuming both interstitial and substitutional sites for the TM. The equilibrium configurations, binding energies, and approximate gap levels of all the {Ti,Hn} and {Ni,Hn} complexes are calculated.
